Share Email Opening Title Case Manager Healthy Families (PT)  Description Looking for a Part-time Case Manager for our Healthy Families Program in Bergen county   Job Summary: Provide in-home education and supportive services to new and expectant parents in Bergen County, especially those families overburden by stressors placing them at risk of child abuse and neglect.   What You Will Do: Conduct on-site or off-site assessments through face-to-face interaction Conduct intensive home visitation for at least one (1) hour to assure parenting skills, child development, child health, and other aspects of family needs are met Track all referrals, screens, and assessments in the HF-TIP database system.
