Share Email Opening Title Case Manager ICMS  Description Looking to hire Case Managers for our Integrated Case Management Services Program (Starting at $19.54 per hour)   Job Summary: You will provide direct case management services to an adult mental health population in the community on a 24/7-hour basis. You will spend 65% of time face-to-face with consumers, in their homes and the community.   What You Will Do: Facilitate and coordinate psycho-educational services for consumers and their family members on a variety of topics relating to wellness , health promotion, prevention, skill-building and recovery topics Offer on-site and outreach services (including transport of consumers) Fulfill case management services, including coordinating or directly facilitating linkages to services, service coordination, and treatment enrollment assistance Ensure consumers have community supports in place that fulfill a positive corroborative role Communicate observations and information about the consumer in the consumer's service record and to team members Participate in supportive counseling and crisis intervention Participate in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans Contribute to formulation of consumer-centered care plans in collaboration with team members, containing relevant, measurable goals and realistic time frames Perform reviews of progress and evaluate effectiveness of goals Work with administrative staff to ensure that all services are documented based on billing and regulatory standards   What We Need to See: Bachelor's Degree in Social Work OR Bachelor's Degree in Mental Health or related field, plus one (1) year post BA experience in Mental Health or related field Valid driver's license insurable by agency's automobile insurance   What We Offer: 401K, no matching needed High plan insurance at a generous cost Union benefits Life and Disability insurance Per diem opportunities Excellent supervision and training Continuing education, tuition reimbursement and eligibility for loan forgiveness 10% daycare discount   About Care Plus New Jersey: CPNJ embraces a culture of diversity and inclusion in all of it policies and procedures.
