Share Email Opening Title Counselor SVS  Description Looking for a Full-Time Counselor for our Supervised Visitation Services Program   Job Summary: You will provide outpatient therapeutic sessions to children, adolescents, and their families to increase family stabilization, and also maintain accurate records for each case to ensure departmental goals are accomplished within prescribed time frame by performing the job responsibilities.   What You Will Do: Evaluate clients at intake, admission, ongoing treatment, service delivery, and at discharge or transfer Create psychosocial treatment plans with relevant, measurable goals that serve as indexes for evaluating treatment progress, and realistic time frames for achievement Provide services and supportive interventions, including case management, linkage to community resources, and provide inter-agency coordination of treatment Respond to clients for crisis intervention Contribute to inter-organizational care coordination Document services in clinical record with all entries authenticated and time/dated Communicate discharge/aftercare/transfer plans for clients completing treatment or accessing other services in the continuum Be a member of an interdisciplinary team in the assessment and evaluation of clients Communicate, in written documentation, the integration of client history and assessment information Report directly to the Program Director/Manager   What We Need to See: A Master's degree in a behavioral health discipline from an accredited college or university 3+ year work experience with children and adolescent population and crisis intervention.
