Case Manager SAIF

Looking for a Full-Time Case Manager for our SAIF program

Job Summary:
You will provide intensive case management services to adult clients on welfare covering several counties, maintain a caseload as assigned, and develop individualized service plans that are consistent with WFNJ and SAIF regulations.

What You Will Do:

  • Complete ongoing evaluations/reassessments through regular contact with clients to appraise level of functioning and progress towards goals. Such contact should include: 1) Home visits 2) Client telephone conferences 3) Client office visits 4) Visits to client work site
  • Communicate client's progress, needs, and concerns to Program Manager through case review and monthly reports
  • Provide crisis intervention and consultation to clients, their families, employers and others while observing program's requirements;
  • Maintain case file on every assigned client and link client to appropriate supportive services
  • Advocate on client's behalf and assists with applications for services and linkage to community services and resources
  • Maintain open communications with all providers, including the Regional County Board of Social Services, the Dept. of Human Services, and Work First New Jersey Regional providers
  • Identify potential employers, develop job opportunities, help place our clients in the competitive work situation, and maintain relationships with potential and existing employers.

What We Need to See:

  • Bachelor's Degree in Social Work or related field from an accredited college or university
  • 1+ years of experience in crisis intervention, counseling, mental health, substance abuse, domestic violence, family therapy, group work and job development
  • Valid driver's license required and must be insurable by agency's automobile insurance
