If you are passionate about what you do and want to use your expertise to engage in meaningful and challenging work, please apply to join our team today.   Description One Full-time contracts and one .5fte (17.5 hrs/wk) contract to March 31, 2023.  (with a possibility of extension or becoming permanent) The Transitions Facilitator is responsible for the development of service plans that focuses on community participation and planning for adult transition for children and youth who have autism and are registered in the Ontario Autism Program. Services and supports also include referrals and linkages to service providers for training, education and/or skills acquisition in an effort to start planning for adulthood. The Transitions Facilitator will work with community partners such as school boards, Developmental Services Ontario and others to assist with transition planning as well as to guide families through the transition process, offer group options for transition support and other information sessions. This will include practical support in confirming eligibility for adult developmental services, linking with DSO CER, and application to the Ontario Disability Support Program (ODSP).
