Assist to navigate the service system and explore all community service options available to them. Tasks will include assessment of needs, development and coordination of service plans, referral support and skill development. As well as working with families that have a connection to York Region CAS that have a risk or imminent risk of family breakdown due to factors such as complex behavior, mental health diagnosis, school/home breakdown, and minimal connections to community supports and services.
