Schedule: Monday - Friday, 10:00 am - 6:30 pm. Occasional evening and weekend work may be required as job duties demand.

What You Will be Doing
We want people to join our team who share our values of love, excellence, respect, empowerment, and well-being. The ideal candidate will be responsible for ensuring that the clients' medically related emotional and social needs are met and maintained on an individual basis in accordance with policies and procedures and current federal, state, and local standards, guidelines, and regulations. The primary responsibility will be providing services for the Youngstown Behavior Health programs.

Primary Job Responsibilities

  • Develop a comprehensive social history and diagnostic psychosocial assessment that includes the client's problems, strengths, and preferences, including cultural, religious, and ethnic background and implications for the care plan.
  • Helps clients and their families identify needs and develop service plans to meet those needs.
  • Orients clients and families to the program, its services, its service limitations, and clients' rights.
  • Involves clients and families in care planning, encouraging their attendance at care conferences.
  • Provides clinical services within the Afterschool program, Adult Day Treatment, and Multisystem Youth programs.
  • Attends scheduled care conferences and identifies and interprets psychosocial needs of clients for inclusion in the care plan.
  • Completes mandatory documentation within timelines established by applicable regulations and policies.
  • Serve as a liaison between individuals, families, and care team members to positively promote collaboration.
  • Coordinate special events/workshops to provide educational opportunities for families/caregivers.
  • Facilitates monthly meetings for families to provide education and provide an opportunity for caregivers to talk about strengths and challenges, discuss issues of concern vent frustrations and learn from others.
  • Provide a safe place for parents and siblings to process emotions and gain additional clinical support.
  • Establish and maintain positive relationships with the local system of care partners such as the children's services system, boards of developmental disabilities, schools, family, and children first councils, managed care organizations, providers of behavioral health services, and primary care providers.
  • Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals, and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.).
  • Provides clients and families with crisis management services as needed.
  • Develops a comprehensive discharge plan in collaboration with clients, clients’ families, multidisciplinary staff, and referral agencies.
  • Trains and consults with staff regarding psychosocial needs that encompass mental health, development, and diversity of individual clients to increase staff knowledge and skills.
  • Educates staff about any relevant state legislation or regulations relating to behavioral health services.
  • Advocates on a case, policy, and program level.
  • Participates in policy decision-making in a way that encompasses an understanding of the issues and implications relevant to the developmental and behavioral health needs of clients served.
  • Meet required productivity requirements of 25 billable hours per week.
  • Provide clinical review and audit of clients' charts and documentation to ensure compliance and support quality improvement.

Minimum Education/Qualifications

  • Bachelor’s degree in social work or a related field.

Preferred Education/Qualifications

  • Master’s degree in social work or related field.
  • Two (2) years of supervised social work experience in a health care setting, working directly with individuals.
  • Experience working with youth with autism spectrum disorders.
  • Experience working with adults and youth with mental health disorders.

Required Licensure/Certification

  • Licensed through the State of Ohio as an LSW/LPC/LISW/LPCC or obtained within six months of the date hired.
  • Ability to obtain and maintain First Aid, CPR - adult, child, and infant. (Training will be provided, if not already certified).
  • Valid Ohio Driver's License with Ohio Bureau of Motor Vehicles; proof of auto insurance.
  • No more than 5 points on license.
  • Within 60 days of hire, successful completion of the required physical crisis management training(s) and certification(s) applicable to the assigned program.

Special Skills and Requirements

  • Must be able to function independently and have flexibility, personal integrity, and the ability to work effectively with residents, staff, and support agencies.
  • Must be able to relate to and work with ill, disabled, emotionally upset, and at times hostile people within the programs.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • COVID Vaccine Required.
