Looking for Full-Time Clinician for our Coordinated Specialty Care program.

 

Job Summary:

The Clinician assists clients with clarifying goals, coping with stressful situations, interacting more effectively with other people, and overcoming barriers to their recovery.

 

What You Will Do:

  • Assess and evaluate adolescent and adult clients for appropriate level of care or programming, including therapeutic phone response & crisis intervention counseling
  • Assist client with concrete service linkages and advocacy
  • Provide and follow up on referrals when indicated by initial and on-going assessment
  • Formulate treatment plans, with relevant, measurable goals that serve as indexes for evaluating treatment progress, and realistic time frames for achievement
  • Provide therapeutic services and supportive interventions, including outpatient therapy, case management, linkage to community resources, and provide inter-agency coordination of treatment for adults, children, and families
  • Conduct crisis assessment and intervention for clients
  • Recognize and address gaps in service and actively collaborate for resolution
  • Communicate and document discharge/aftercare/transfer plans for clients by completing treatment and accessing other services in the continuum

 

What We Need to See:

  • Master's Degree in social work or related field
  • A provisional or professional clinical licensure in NJ (LSW, LAC, LCSW, LPC, LMFT)
  • At least 1-2 years of clinical work experience with families, children and adolescent population
  • A valid driver's license and must be insurable by Agency's automobile insurance
