Definition

Promotes health and wellness in the community through the provision of professional nursing care at HealthWorks for Northern Virginia including patient triage, direct patient care and patient education. Provides coordination of care, oversees the smooth day to day operations of the clinic floor as it affects the clinical staff and clinical services and provides direct supervision of his/her team of nursing staff.

Roles & Responsibilities

As a member of the health care team, working under the supervision of the Director of Nursing. The duties of the LPN Charge Nurse include:

Patient Care

  • Assessing patient needs based upon physical exam and health history.
  • Performing diagnostic tests as ordered by the provider staff.
  • Administering medications and vaccines as ordered by the provider staff or as indicated by standing orders and protocols.
  • Acts in the capacity of the triage nurse or float nurse when needed.
  • Provides health and wellness education for all clients delivered in a culturally competent manner.
  • Assists clinical teams with management of high-risk clients and emergencies.
  • Demonstrates an appropriate knowledge of the meaning of informed consent and ensures that informed consent is obtained for procedures as appropriate per protocol.
  • Educates patients and family members on the patient’s diagnoses within scope of knowledge and authority.

Clinical Skills

  • Demonstrates ability to carry out skills appropriate to patient population served within scope of practice.
  • Demonstrates ability to use equipment according to nursing procedures and as used in the department.
  • Demonstrates ability to assist providers with treatments as appropriate.
  • Demonstrates ability to carry out procedures following established standards of care.
  • Demonstrates the ability to care for patients as assigned.
  • Always treats patients and their families with dignity and respect.
  • Demonstrates ability to interpret vital signs, and responds appropriately, reporting abnormalities to the provider.
  • Demonstrates clinical competence by correctly performing required skills to include suture and staple removal, injections, blood and urine samples, and other skills associated with LPN level nursing.
  • Demonstrates an appropriate understanding of medications and pharmaceutical aspects associated with drug considerations.
  • Serves as a mentor/example in quality patient care and adherence to clinical protocols for other clinical staff
  • Performs various duties to ensure quality patient care, excellent patient flow, and efficiency of operations within the nursing department.
  • Participates in Center projects, initiatives, and/or committees as requested.
  • Monitors medical supplies to ensure that the department has the appropriate materials to provide efficient and effective service.
  • Completes appropriate paperwork in a timely and legible manner.
  • Ensures that appropriate consent forms are signed, and reviews forms associated with different programs to ensure that all paperwork is in order.
  • Serves as clinical staff resource in dealing with patient triage and other difficult patient issues that may arise.
  • Serves as clinical staff resource and ensures that excellent patient flow is maintained by working with and helping other members of the clinical team as needed.
  • Participates in Quality Improvement Program and does quality monitoring as assigned.
  • Is proficient in documentation and use of the EMR, eClinical Works, (eCW). Is willing and able to assist other clinical staff as needed in the use of eCW.
  • Willingly serves as a preceptor to newly hired clinical staff as needed.
  • Demonstrates professional development and clinical competency.

Administrative Duties

  • Performs various duties to ensure quality patient care, excellent patient flow, and efficiency of operations within the nursing department.
  • Posts daily assignments and develops monthly nursing staff schedule to ensure appropriate provider coverage and safe, high quality patient care.
  • Participates in Center projects, initiatives, and/or committees as requested.
  • Monitors medical supplies to ensure that the department has the appropriate materials to provide efficient and effective service.
  • Completes appropriate paperwork in a timely and legible manner.
  • Keeps professional licensures and certifications active, status and renews timely.

Supervisory Skills

  • Supervises the administration of medications and/or vaccines by ancillary staff.
  • Responsible for nursing staff scheduling to include nurse reassignments, callouts, and ensuring adequate coverage.
  • Responsible for new nursing staff orientation.
  • Responsible for ensuring that equipment and supplies are in stock and updated.
  • Acts as a mentor/coach to the nursing staff including conducting in-services and other educational sessions as assigned.

Supervisory Responsibilities – Oversees and supervises the workflow of Medical Assistants and floor nurses

 
Position Requirements

Qualifications & Education Requirement

  • Current Commonwealth of Virginia License for Practical Nursing from an accredited institution.
  • Minimum of two years clinical experience at an acute care or ambulatory setting preferred.
  • Supervisory experience preferred.
  • Community or public health experience highly preferred.
  • BLS required.
  • English/Spanish fluency preferred.
