Duties include: communicating with new potential clients to explore what services are needed, managing a caseload of caregiver clients, which includes supportive counseling, information and referrals, advocacy, and other services, including determining eligibility for benefits and entitlements (such as Medicaid and SNAP), on an ongoing basis, facilitating support group meetings, and making and sustaining connections at community establishments, such as healthcare facilities, houses of worship, and other social service agencies.   Position Requirements Spanish or Haitian Creole-speaking applicants strongly preferred, COVID vaccine required, and occasional home visits might be required.   This position is currently not accepting applications.
