NFMH Liaison

Wyandot BHN is seeking a dedicated NFMH Liaison to join our Wyandot Center team! The NFMH Liaison will provide liaison services to those currently residing in or those who are in need of services provided at a Nursing Facility for Mental Health.  

Accountabilities:

  • Conducts PASRR-Level 2 assessments for individuals referred for NFMH placement
  • Works closely with the referring party and the individual to complete the assessment
  • If approved for NFMH level placement, the liaison will work with the referring facility and area NFMHs to obtain placement and coordinate admission to a NFMH
  • Conducts Continued Stay Screens and Resident Reviews as needed
  • Works closely with the NFMH staff to make recommendations for on-going care and needs
  • Coordinates and/or provides Specialized Services for individuals identified in the NFMH to help determine readiness, desire and needs to discharge from NFMH to return to the community.  This includes, but is not limited to, coordinating care with other providers and/or providing direct care and services to the individuals in the NFMH.
  • Works closely with the NFMH when a person has been identified as ready to discharge from the NFMH to coordinate a discharge plan including connecting the individual with on-going treatment providers in the community.
  • Works closely with community partners and/or entities to problem solve complex situations and develop plans for immediate and appropriate interventions in the community.
  • Provides crisis interventions on-scene in the NFMH if needed.
  • Coordinate and facilitate referrals to appropriate services in the community and under our Wyandot Behavioral Health Network umbrella, including RSI
  • Understand and follow the guidelines and policies of the Wyandot Center work within the community partner guidelines/policies.
  • The liaison will document all contacts and interventions in the EHR in a timely and accurate manner as well as track other required data points as needed.
  • Contributes positively to the development of new programming and demonstrates solution-focused leadership abilities.
  • Works closely with other service providers and case management teams to provide clinical assessments and interventions for consumers who are involved in and/or referred for services
  • Adheres to and remains compliant with Medicaid, State licensing regulations, grant and other contractual standards.

Qualifications:

Education:   

  • Master’s degree in social work, psychology, counseling or other mental health related field. 
  • Must be licensed as Qualified Mental Health Professional through the Kansas Behavioral Sciences Regulatory Board.

Experience:

  • Must have familiarity with Trauma-Informed systems of care.
  • Preferred experience with working in a structured setting
  • Preferred training to complete PASRR assessments.

Skills & Abilities:

  • Excellent interpersonal and communication skills, including both oral and written.
  • Must possess strong team skills with a positive spirit, high energy and a “can-do” personality.
  • Relevant experience in mental health crisis services and community resources.
  • Ability to recognize and be sensitive to cultural and ethnic differences.
  • Ability to establish effective working relationships with personnel from multiple systems, including hospitals, nursing facilities, state entities, etc…
  • Ability to facilitate planning meetings with individuals from multiple systems.
  • Proficient in use of technology, including Microsoft Office and use of an EHR.
  • Provide and maintain dependable transportation and auto insurance.
  • Ability to build and maintain close working relationships with clients and community partners.
  • Ability to provide direction for programming and planning purposes.

Work Conditions and Considerations:     

Provides assessments in multiple locations including the community, hospital settings, nursing facilities and via telehealth. Travel within the community is required including transporting clients as needed.

 
Position Requirements

Job Requirements:

  • Master’s level degree in social work, counseling, or related field required.
  • Must have licensure level to be a Qualified Mental Health Professional (QMHP).
  • Familiarity with mental health theory and practice.
  • Ability to recognize and be sensitive to cultural, ethnic and socioeconomic differences.

 

 
Full-Time/Part-Time Full-Time  
Hours Monday - Friday, 8:00 am to 5:00 pm
