Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

As a member of the Mental Health Case Management team, the Case Manager for Permanent Supportive Housing (PSH) provides case management services to the long-term mentally disabled population within Staunton, Waynesboro, Augusta County, and Highland County. The incumbent collaborates with teammates to enhance psychiatric stability, quality of life, access to available resources, and appropriate use of medical services and inpatient care.

Essential functions of the position include:

  • Conduct comprehensive initial and annual assessments of client strenghts and needs.
  • Understand and embrace the Housing First principles and engage in a housing-focused case management approach to service delivery.
  • Assist clients in obtaining and maintaining entitlements and other resources for support.
  • Proactive and ongoing client assessment, outreach, and advocacy.
  • Work alongside clients to develop and administer person-centered treatment plans.
  • Provide crisis intervention support and coordination for individuals experiencing acute distress.
 
Position Requirements

Minimum Requirements:

  • Bachelor's Degree in Human Services or related field.
  • One year of direct clinical experience providing developing, conducting, and approving assessments and individual service/treatment plans for individuals with serious mental illness (SMI).
  • QMHP certification or QMHP Trainee eligible.
  • Valid Virginia driver's license with an acceptable driving record.

Knowledge, Skills, and Abilities:

  • Knowledge of:
    • Basic human needs.
    • PSH model of care.
    • Housing First Principles.
    • Human personality growth and development.
    • Mental health and co-occurring disabilities.
    • On-going evaluation of concurring mental illness and substance use disorder (SUD).
    • Supportive counseling techniques, including individual, group, and family as it relates to individuals with mental illness and SUD.
    • Crisis intervention.
    • Psychosocial rehabilitation.
    • Clinical case management services.
    • Local and state community resources.
    • Interagency services coordination.
  • Skills pertaining to:
    • Evidence-based counseling techniques and theories of counseling.
    • Individual, group, and family counseling.
    • Mental status assessment, treatment planning, intervention implementation, and evaluation of an individual's needs.
    • Treatment recommendations in accordance with PSH model.
    • Emergency or crisis intervention services.
    • Mental health and SUD consultation and education services.
    • Care coordination.
    • Psychosocial rehabilitation services for individuals with SMI.
  • Ability to:
    • Engage and support individuals with SMI and co-occurring disorders.
    • Implement interagency coordination of services for individuals in need of services.
    • Work cooperatively with Valley CSB colleagues involved in the provision of outpatient services.
    • Exercise sound judgment.
    • Demonstrate initiative.
    • Maintain confidentiality and operates with high ethical standards in accordance with industry best practices.
    • Participate in a rotating on-call shift to provide immediate assistance outside of office hours.
    • Manage appointments while maintaining flexibility in order to accommodate individuals as needed.
    • Maintain a professional demeanor with colleagues and individuals receiving services at all times.
    • Contribute to a healthy organizational culture through a positive attitude and dedication to the VCSB Mission
    • Work within a dynamic team environment.
    • Occasionally work at times when VCSB is closed during regular business hours (e.g., inclement weather).
    • Operate with autonomy and as part of a team as necessary.
