ROUTINE DUTIES: Responsible for providing quality care, activities, and personal care to adults with disabilities Maintains records of services performed and of apparent condition of resident/participant. Works with Supported Living Manager to develop and coordinate behavior plans and outcome measurements for residents/participants. Maintains a cooperative relationship with families and guardians of residents/participants, as well as with team-members within your department and other agency departments, and with all collaborating community agencies and service providers. Assures all charting is up-to-date and signed off on in resident's binders.
