Description

One Part-time (17.5 hr/wk) Permanent Position

In this role, you will work as part of the Safe Beds Program to provide comprehensive and collaborative community based case management response to individuals aged sixteen and over who are guests of the YSSN Safe Beds Program. Utilizing a Recovery Model framework, the Mental Health Case Manager will provide mental health and addictions support to residents of the program, during their 30 day stay. The Mental Health Case Manager will assist Safe Beds guests in identify needs and goals that will enhance the quality of their lives, this may include: conducting needs assessments, providing mental health and addictions education, making outbound referrals, liasing with community agencies and services, and teaching ADL's".

Position Requirements
  • Bachelors degree in Social Work or a related discipline (i.e. Health Sciences.etc)
  • Minimum 3 years related experience required
  • Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills are assets
  • Counselling, crisis prevention and intervention experience
  • Experience working with individuals having serious mental illness is essential
  • A second language is considered an asset
  • The successful applicant will be required to provide a police clearance that meets agency standards
  • A valid G driver’s license, business use insurance and regular use of a reliable vehicle are required for this position
