Therapist - Intake Specialist - NFMH Liaison - Adults

Wyandot BHN is seeking a dedicated Mental Health Therapist to add to our Community Support Services, Homeless Team. This a full-time position with two roles: 1) Intake Specialist, and 2) NFMH Liaison. The Intake Specialist completes assessments to determine if persons in a housing crisis meet criteria for service. The NFMH Liaison role completes assessments on consumers in Wyandotte County seeking placement in a Nursing Facility for Mental Health.

Essential Job Duties:

Intake Specialist duties include:

  • Complete intakes for homeless consumers to access needed mental health and homeless services, and make appropriate internal and external referrals.
  • Provide back up and clinical support to homeless case managers and outreach specialists when appropriate.
  • Assist with program data collection, grant writing and other administrative needs as assigned.
  • Facilitate therapeutic interventions for clients experiencing emotional distress.
    NFMH Liaison duties include:
  • Coordinate information and services between Wyandot Center and NFMH/hospital that relate to a consumer's admission, ongoing treatment, discharge and outpatient follow-up.
  • Conduct PASRR Level II assessments to determine appropriateness of nursing home placement for consumers.
  • Conduct Continued Stay screens on all Wyandotte County residents and/or other NFMH residents seeking independent living in Wyandotte County.
  • Refer client and/or family to community resources for housing or treatment to assist in recovery from mental or physical illness.

 
Position Requirements

Job Requirements:

  • Master’s level degree in social work, counseling, or related field required.
  • Must have licensure level to be a Qualified Mental Health Professional (QMHP).
  • Familiarity with mental health theory and practice.
  • Ability to recognize and be sensitive to cultural, ethnic and socioeconomic differences.
