Description

Valley Community Services Board (VCSB) is looking for an Intercept 2 Rural Jail Rapid Diversion Case Manager to join our team! Under the direction of the Rural Jail Rapid Diversion Clinician the Case Manager will provide services to individuals who are identified as having a Serious Mental Illness and who have become judicially involved. 

This position is a 30 hour per week (75%) position.

Essential Functions

  • Provide case manager services to those individuals who are identified as having a Serious Mental Illness (SMI) and have become judicially involve.
  • Work closely with Blue Ridge Court Services Unit to identify potential participants.
  • Coordinate the assessment of individuals for eligibility/appropriateness for VCSB intake process.
  • Expedite engagement with clinical services, facilitate engagement of services and coordinate with VCSB Rapid Diversion Clinician.
  • Monitor process/engagement including reporting outcomes for a one-year period post engagement.
  • Case Manager will participate in Court procedures. serve on the Therapeutic Docket Steering committee; coordinate clinical service delivery.
  • Case Manager will also provide consultation to Diversion team members (Court/BRCSU/Blue Ridge Criminal Justice Board and VCSB).
  • Case Manager will exercise considerable independence and autonomy in day-to-day decision making.
 
Position Requirements

Minimum Requirements: This position requires expertise in case management and coordination of community resources and the ability to work effectively with other agencies/referral sources, and inpatient facilities. 

  • Requires graduation from an accredited college or university with a minimum of a bachelor’s degree in Human Services Field

Preferred Qualifications:

  • Qualified Mental Health Professional (QMHP) preferred but not required.
