Definition
As a member of the health care team, under the supervision of the Director of Nursing, he/she will be responsible for managing the care of a defined patient population with a focus on diabetes, hypercholesterolemia, and obesity. This will include developing culturally competent expected patient outcomes, identifying barriers, and working collaboratively with providers and other care team members along with the patient and their family to adopt achievable self-management goals. The ideal candidate will have the ability to work as a team member in a very fast paced, high energy setting. This position must be able to maintain a positive, supportive relationship with the clinical team, patients and their families. Must maintain confidentiality as appropriate.
Roles & Responsibilities
- Works as part of the clinical team assessing patients; reviewing lab reports and medical records, confers with medical providers and other team members to identify and engage patients and their families in need of nutritional counseling.
- Meet directly with patients and their families to identify educational needs; provide individual instruction, utilize informational handouts/booklets, or refer to community resources as needed.
- Conduct routine screening to create a tailored, culturally appropriate disease prevention and treatment plan to address nutritional problems in children and adults, within scope of practice.
- Establish treatment plans and communicate with patients and the clinical team regarding program compliance.
- Adapting and/or developing interventions and teaching materials to help reduce the risk of comorbidities such as diabetes, heart disease, hypercholesterolemia, obesity, or treating these complications.
- Find creative solutions to patient barriers and needs that may require innovative thinking to locate the appropriate resource(s) to fit patients’ lifestyles and financial resources.
- Participate in the Quality Improvement Council and provide input and recommendations regarding barriers to care and how to address equity challenges.
- Collaborates with the Director of Clinical Data Analytics and the Quality Improvements Manager to implement monitoring and evaluation tools to assure effectiveness of programs and meet funding source requirements.
- Attend relevant continuing education events and training as identified and approved by direct supervisor.
- Perform other related duties as assigned.
