LPN - Team Leader Full-time Evenings 

Position Details LPN - Fulltime  
Shift Hours 3:00pm - 11:30pm  
Number of Bi-weekly Hours 80 hours (10 days per biweekly pay period)  
Days of the week Varies  
Weekends/Holidays Every other Weekend and Holiday

The LPN - Team Leader will support the Frederick Living Mission of caring for and enriching the lives of older adults with the following essential responsibilities:

  • Observe and report changes in resident condition to the Nurse Supervisor or physician/ physician extender.
  • Communicate with Supervisor, ADON, DON, NHA, care providers and family/ POA as needed.
  • Uses evidenced-based practices to ensure care delivery is provided to residents.
  • Assist in the management of personnel through on-going performance evaluation, Care Chats, coaching, counseling, education, and Corrective Action as needed.
  • Complete all required documentation accurately and timely.
  • Participate in resident TOC/ Care Conferences and care plan review as needed.
  • Collaborate with Cedarwood staff to ensure optimal care for residents.
  • Comply with Infection Control practices.
  • Maintain hand-held communication equipment is maintained by all direct-caregivers for effective call bell management & communication with team members.
  • Attend and complete all required in-services, training and Team Meetings.
  • Abide by the organization’s code of conduct in the performance of their duties.
  • Perform other duties as required.

  • Ability to hear, feel, see, smell and speak adequately to perform essential functions of the job.

  • Ability to operate in a high stimulation environment.

Along with an excellent work ethic, successful candidates will have:

  • Licensed Practical Nurse in the Commonwealth of Pennsylvania.
  • Prior experience in geriatric nursing preferred.
  • High school diploma or equivalent.
  • BLS Healthcare Provider.
  • Basic computer skills
