Description

VCSB is looking for Forensic Mental Health Case Manager (MHCM) to join our team! As a member of the Adult Mental Health Case Management team, the Forensic MHCM provides support to individuals transitioning into the community from incarceration.  This is a highly collaborative position that partners with Middle River Regional Jail and community organizations throughout Augusta County, Highland County, Staunton, and Waynesboro.

Essential Functions

  • Provides referrals to other agencies, supportive counseling, outreach, crisis intervention, and assessment.
  • Carries out client treatment plans developed from initial & ongoing assessments and clinical staffing.
  • Assists clients in obtaining and maintaining disability and social entitlements and any associated benefits.
  • Monitors and coordinates with DAP liaison to maintain DAP benefits.
  • Monitors and coordinates with VCSB Rep Payee to manage the consumers budget and personal needs as appropriate.
  • Advocates and promotes recovery-oriented practices.
  • Ensures Service Plans reflect client strengths, need and interest.
  • Ensures service goals promote client self-determination and independence.
  • Utilizes office based and or community visits to provide support counseling and case management services.
  • Initiates and maintains collateral contacts with other treatment providers within in VCSB as well as outside the agency.
  • Provides consultation and coordination of care with family and other providers.
  • Provides outreach to clients via home visits, telephone calls or meetings in the community.
  • Assists VCSB crisis team in providing crisis intervention services when clinically appropriate.

Knowledge of:

  • Basic human needs.

  • Interviewing techniques.
  • Appropriate professional behavior.
  • Basic psychopathology.
  • Basic crisis intervention techniques.
  • Effective intervention strategies for clients with serious mental illness.

Skills pertaining to:

  • Assessment of a client's ability to adequately maintain themselves in the community.
  • Helping clients identify appropriate resources for support.
  • Assessing the need for hospitalization.

Ability to:

  • Advocate for clients.
  • Intervene in a crisis.
  • Work effectively with adults with a wide range of mental disorders and co-occurring challenges.
  • Organize and work effectively.
  • Communicate clearly in written and oral formats.
  • Demonstrate sound professional judgment.
  • Build and maintain collaborative relationships within and outside VCSB.
  • Maintain high professional ethical standards and appropriate confidentiality.
 
Position Requirements

Minimum Requirements

All candidates must meet the following requirements in order to be considered for the position.

  • Bachelor's degree in human services, criminal justice, or related field.
  • Previous work or internship experience that provides the necessary knowledge, skills, and abilities.
  • Must pass required pre-employment screenings, including, but not limited to, criminal history and experience verification.
  • Current Virginia driver’s license with zero points or more.

Preferred Qualifications

The most competitive candidates will meet one or more of the following criteria, but these preferred qualifications are not required to be considered for the position.

  • Qualified Mental Health Professional (QMHP) or Licensed Mental Health Professional (LMHP) certification preferred but not required.
  • Previous experience serving adult clients with mental illness.
  • Previous experience with the criminal justice or corrections systems.
