Seeking a Counselor for SVS Essex (Supr. Visit Svc.)

Summary of Position – Provides all needed services to consumers based upon the level of functioning and disability and the consumers’ needs.Maintains confidentiality and immediately reports to management breaches in confidentiality.

  • Observes and maintains appropriate professional boundaries with clients, co-workers and supervisorsed
  • Completes all required documentation/paperwork accurately and within established time-frames
  • Knowledgeable of, and practices the organization’s health and safety policies and procedures, including but not limited to fire safety and emergency preparedness, infection control procedures and incident reporting
  • Offers on-site services and support appropriate to meet the consumers’ needs
  • Communicates observations and information about the consumer to supervisor(s)
  • Provides crisis intervention services, but not crisis intervention counseling
  • Assesses and/or evaluates consumers and families at intake and/or admissions, during ongoing service delivery and at discharge; collaborates with treatment providers to establish comprehensive assessments and service plans
  • Participates in individual service coordination activities as defined in N.J.A.C. 10:37A-1.2;
  • Performs the following duties as a member of an interdisciplinary team (internal and external) - conducts outreaches, assesses consumers and articulates (verbally and in written documentation) the integration of consumer history and assessment information; mentors new staff
  • Collaborates as a member of an interdisciplinary team, including but not limited to active participation in program and team meetings, provides support for team colleagues, and engages in program-related performance improvement processes and activities
  • Engages consumers in services and accesses and/or addresses their needs; assesses medication compliance; includes client feedback in all interventions and utilizes a Strengths based perspective; provides comprehensive services
  • Formulates service plans containing relevant goals, behaviorally measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement
  • Documents observations, information and services provided to the consumer
  • Records/documents the following information in the consumer’s clinical record; assessments, service plans, progress notes (DAP format) and discharge summaries, with all entries authenticated and dated
  • Participates in quality assurance activities and cross-functional action teams, as directed
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data
  • Safely operates and utilizes Agency vehicles while transporting clients to and from specialized medical appointment; follows all policies and procedures related to vehicle safety and maintenance as outlined in the Agency’s Transportation Manual
  • Contribute to the development, implementation, monitoring, and update of the individualized rehabilitation plan
  • Perform therapeutic rehabilitative skill development
  • Offer services appropriate to meet the consumers’ needs
  • Perform crisis intervention
  • Ability to assess history, assessment, family information, in the context of adult development in formulating service plans
  • Ability to engage family members in decision-making related to adult service planning
  • Knowledge of psychotropic medications and their use prescribed for adult populations

 Qualification 

A bachelor’s degree in one of the helping professions (e.g. social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice), OR

  • Bachelor’s degree in a field other than a helping profession AND 2 year experience in a community behavioral healthcare setting
