Population Health Manager

As a pivotal role in health outcomes for our Communities, the Population Health Manager will utilize population health management and quality improvement initiatives to improve overall patient outcomes, access to care, and value-based care management.

 

Key functions:

  • Manage payor and value based contracts
  • Oversee patient population
  • Lead a team of Population Health Professionals including clinical support staff
  • Maintain data and metrics that align with regulatory agencies (TJC, PCMH, FTCA, etc)

 

Requirements:

  • Bachelor's degree or equivalent experience. Coursework in Lean Six Sigma or Data Management is a plus.
  • 5+ years of experience in a medical office setting
  • Available during regular business hours of 8am-5pm, Monday-Friday, with occasional evenings and Saturdays.
  • Travel throughout the Quad Cities Metro Area for events, meetings, etc.
