Share Email Opening Title Cognitive Care Navigator  Description OCCK is looking for a highly engaged, detailed oriented, self-motivated, advocacy minded individual to join our growing Kansas Community Care Ecosystem Partnership. Help us connect with health professionals, educate, empower and build supports for rural Kansan’s who are experiencing early symptoms of changes in the brain. This is a full time professional position, competitive wage, with a great benefits package including KPERS. The schedule is Monday thru Friday 8am-5pm with some flexibility. It is based out of Salina Kansas with driving throughout our coverage region to provide outreach and education. Mileage reimbursement or a company vehicle will be provided during required travel. Must be able to work well with aging adults including those with an I/D disability and their caregivers .   Position Requirements Four-year degree in human services field such as social work, psychology, social sciences, or interpersonal work experience. Works independently with initiative and efficiency. Skilled at organization and task analysis. Demonstrates confidence and leadership in challenging situations. Demonstrates a persistently positive and creative approach to problem-solving. Knowledgeable regarding community resources. Provides excellent customer service. Excellent computer and organizational skills.
