The Outpatient Case Manager assists patients with inquiry calls, coordination of care and discharge planning. Includes gathering patient information, setting follow up appointments, facilitating transition of care to other providers and networking with agencies to better understand their services and referral processes. Additionally the Social Services Case Manager has responsibility for ensuring the proper utilization of hospital resources for patients admitted to the hospital. This position may require some evening and weekend hours as needed to meet patient needs.
Education: Master's Degree from an accredited college or university, in Social Work, Counseling or a clinical related mental health field.
Experience: Therapist I: One (1) year experience, but will consider new grads in an entry-level capacity with a supervisory plan. Therapist I: State certified in social work or a clinical related mental health field (LCSW) or (LPC) within two (2) years of employment preferred.
Additional Requirements: CPR certification and Crisis Prevention (CPI) during orientation and prior to assisting in a restraining procedure. May be required to work flexible hours and overtime.
Chemical Dependency Specific Requirements: Experience with the provision of individual, family,
and group therapy services required. Must be familiar with chemical dependency issues and have knowledge of 12-step programs and philosophies.
