Description

JOB SUMMARY: The Behavioral Health Care Coordinator is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The Behavioral Health Care Coordinator is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.

QUALIFICATIONS:

JOB SPECIFIC CORE COMPENTENCIES:

  • Assesses clients appropriately by obtaining mental health history and evaluating presenting illness.
  • Order/performs diagnostic testing appropriately.
  • Documents treatment plan appropriately in the EMR.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Screen, assess, and educate patients for common mental health and substance abuse disorders. Facilitate patient engagement, treatment options and follow-up care, within the primary care practice.
  • Conducts comprehensive, culturally sensitive psychosocial assessments, develops care plans, and provides counseling and crisis intervention services to individuals and/or families.
  • Support psychotropic medication management as prescribed by medical providers, and track and document responses in the EMR focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
  • Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider.
  • Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
  • Collaborate with Patient Care Liaisons, Chronic Care Management, and healthy365 Support Navigators to align goals relating to medical, social-emotional needs, and to address social determinants of health.
  • Provides social work consultation to clinic staff and community providers as appropriate. Serves as liaison to community health care network.
  • Other duties as assigned.

Performance Requirements:

Knowledge:

  • Knowledge of health care field and medical office protocols/procedures.
  • Knowledge of specific assisting tasks related to particular medical practice.
  • Knowledge of information that must be conveyed to patients and families.

Skills:

  • Skill in performing operations on Excel database.
  • Skill in diplomacy and interpersonal interactions.
  • Skill in understanding patient education needs by effectively sharing information with patients and families.

Abilities:

  • Ability to learn and retain information regarding patient care procedures.
  • Ability to project a pleasant and professional image.
  • Ability to plan, prioritize, and complete delegated tasks.
  • Ability to demonstrate compassion and caring in dealing with others.

Equipment Operated:

  • Standard medical exam/office equipment, which may include computerized health information management system for medical records, etc.

Work Environment:

  • Combination of medical office and exam/procedure room settings.
  • Well-lit, well-ventilated, adequate space.

Mental/Physical Requirements:

  • Must be able to use appropriate body mechanics techniques when making necessary patient transfers and helping patients with walking, dressing, etc.
  • Must be able to lift up to 30 pounds of supplies.
  • Occasional stress from dealing with many staff and patients.
 
Position Requirements

CERTIFICATION/LICENSE:

  • Licensed Mental Health Counselor/Professional Counselor, Licensed Clinical Social Worker, Registered Nurse with completion of BSN, Licensed Psychologist.

ADDITIONAL LICENSURE/CREDENTIAL REQUIREMENTS:

  • Bachelor of Nursing
  • Masters-level licensure candidate/trainee (e.g. LMSW).

ADDITIONAL EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Demonstrated ability to collaborate and communicate effectively in a team setting.
  • Ability to maintain effective and professional relationships with patients and other members of the care team.
  • Ability to effectively engage patients in a therapeutic relationship, when appropriate.
  • Experience with assessment and treatment planning for common mental health and/or substance use disorders.
  • Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
  • Mandatory Continuing Education: Customer Service, Fire and Safety, Corporate Compliance (including Confidentiality), Infection Control, and education required by regulatory, accreditation bodies, scope of practice, and/or Hancock Physician Network.
