Clinician -crossroads  


Seeking to hire a Full-Time clinician for the Crossroads to Wellness Department

"Clinician will be a member of the Crossroads to Wellness team, providing individual, family, and group therapy to clients in crisis and in need of rapid access to intensive services and will be the primary provider for clients referred by parole, requiring wrap-around services. Experience working with clients from a forensic setting is preferred."

Job Summary:

  • Provide individual, family and group counseling services
  • Facilitate psycho-educational groups for consumers on a variety of topics relating to wellness and recovery
  • Offer on-site as well as outreach services (including transport of consumers)
  • Serve as the team lead in completing intake/assessments and formulating Recovery Plans
  • Fulfill case management services, including, but not limited to direct linkages to services, service coordination, treatment enrollment assistance, and so forth
  • Communicate observations and information about the consumer in the consumer’s service record and to team members
  • Engage in supportive counseling
  • Assess/evaluate consumers at intake/admissions, during ongoing recovery/service delivery and at discharge
  • Collaborate with treatment providers to establish comprehensive assessments and Recovery Plans
  • Formulates Recovery Plans containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data
  • Participates in quality assurance activities and cross-functional action teams
  • Perform other program-specific functions as assigned by the Program Manager such as, marketing within the community, maintaining staff schedule, curriculum development for group sessions, payroll, supervising other program staff

  • Master’s Degree in social work or related field from an accredited college or university and experience providing clinical services to adults with serious and persistent mental illness
  • LCSW, LPC, LSW, LAC required
  • A valid driver's license and must be insurable by Agency's automobile insurance
