Seeking to hire a Part - Time Counselor for the Family First Program Location: Fair Lawn  Job summary Provides in-home services and supportive interventions including: case management, linkage to community resources, and provides inter-agency coordination of treatment Conducts assessment/evaluation of clients at intake, during ongoing service, delivery, and at termination Formulates relevant goals with behaviorally measurable objectives that serve as indices for evaluating intervention progress and realistic time frames for achievement Prepares written documentation of assessments, interception plans, progress notes, and discharge summaries in the clients’ files with all entries authenticated and time/dated Plans for, communicates, and document aftercare plans for clients completing program and/or accessing other services in the continuum Functions within an interdisciplinary team in the assessment/evaluation of clients, and to articulate verbally and in written documentation the integration if client history and assessment information Collaborates as a member of an interdisciplinary team, including but not limited to, active participation in program and team meetings, providing support to team colleagues, and engaging in program- related performance improvement processes and activities and providing leadership within team and functions Participates in weekly team case conferences and trainings as per Family Preservation Services’ requirements Provides 24/7 coverage to caseload   Qualifications Bachelor’s degree or a Master’s degree in Social Work OR other behavioral health discipline from an accredited college or university and two (2) years experience providing counseling to children and families A valid driver's license and must be insurable by agency's automobile insurance.     CPNJ embraces a culture of diversity and inclusion in all of it policies and procedures.
