Seeking to hire a Full-Time Case Manager for the SAIF program

Job summary

  • Maintains a caseload as assigned by manager; provides supportive case management services to adult clients on welfare develops individualized service plans, which are consistent with WFNJ regulations
  • Completes ongoing evaluations/reassessments through regular contact with clients to appraise level of functioning and progress towards goals. Such contact should include but not limited to: 1) Home visits; 2) Client telephone conferences; 3) Client office visits; 4) Visits to client work site
  • Communicates client’s progress; provides crisis intervention and consultation to clients, their families, employers and others in accordance with program’s requirements; consults with supervisors (or designee) regarding each situation before taking action; provides client information to Program Manager for preparation of Monthly level of Service Report. This information is submitted to Program Manager in a timely fashion
  • Maintains case file on every assigned client and links client to appropriate supportive services in cases where client is deemed appropriate for Social Security disability benefits and/or medical deferment
  • Advocates on client’s behalf and assists with applications for services, when applicable; assists with client linkage to community services and resources, as needed
  • Transport clients, as needed
  • Maintains open communications with all providers (i.e. including and not limited to Regional County Board of Social Services, Regional County One- Stops Regional County Dept. Human Services Office, etc.), as well as other Work First New Jersey Regional providers
  • Participate in weekly team case review meetings with supervisor, as directed; participates in monthly case management meetings where Case Manager advocates for clients and program during such meetings; Responsible for assisting Program Manager with monthly presentations, as directed and educates staff and clients in regards to the programs services
  • Identify potential employers and develop job opportunities related to the client’s specific needs. This includes placing the client in the competitive work situation, and maintaining relationships with potential and existing employers. In addition to providing ongoing support for one year following client’s placement in unsubsidized employment
  • Participates in monthly case management sponsored by BCBSS and/or other Regional County WFNJ collaborative meetings
  • Advocates for clients and program during such meetings
  • Responsible for assisting Program Manager with monthly presentations at the Workforce Center/One Stop Centers as directed
  • Communicates client progress, needs and concerns through case review and monthly reports
 
Position Requirements

Requirements

  • Bachelor’s Degree in Social Work or related field from an accredited college or university with experience in crisis intervention, counseling, mental health, substance abuse, domestic violence, family therapy, group work and job development
  • Valid driver's license required and must be insurable by agency's automobile insurance
