One Full-time contracts and one .5fte (17.5 hrs/wk) contract to March 31, 2023. (with a possibility of extension or becoming permanent)
The Transitions Facilitator is responsible for the development of service plans that focuses on community participation and planning for adult transition for children and youth who have autism and are registered in the Ontario Autism Program. Services and supports also include referrals and linkages to service providers for training, education and/or skills acquisition in an effort to start planning for adulthood.
The Transitions Facilitator will work with community partners such as school boards, Developmental Services Ontario and others to assist with transition planning as well as to guide families through the transition process, offer group options for transition support and other information sessions. This will include practical support in confirming eligibility for adult developmental services, linking with DSO CER, and application to the Ontario Disability Support Program (ODSP).
A hybrid remote work arrangement is available for this position.
Bachelors degree in Social Work or related discipline (ie Psychology etc.)
Minimum three years related experience
Experience working with multiple special needs children, youth and their families
Completion of referral forms, the provision of information/resources both generic and specialized and to facilitate access to all children’s services
Experience with developing an integrated service plan
Experience working with team members from multiple children’s sectors
Experience with team facilitation and leadership
Francophone or second language is considered an asset
Understanding of cultural diversity and second language skills are considered assets
The successful applicant will be required to provide a police clearance that meets agency standards
A valid driver’s license, adequate insurance coverage for business purposes and regular use of a reliable vehicle
