Summary:
Responsible for patient outreach and care coordination in the ambulatory practice setting. Provides assessments, interventions, care plans and ability to identify barriers for our patients. Works collaboratively with the care team and achieves program goals. The program will focus on the pediatric and adult population.
Responsibilities:
- Provides assessment and behavioral health diagnosis, care plans, interventions, and referrals to community resources within the scope of practice
- Interviews patients and patients support system using Motivational Interviewing techniques to complete assessments and design appropriate care plans for the patient
- Serves as a patient advocate
- Provides supportive counseling and education to the patients and the patients support systems
- Identifies plans and coordinates a plan of care based on the social and psychosocial needs of the patient and their family/support system
- Provides psychotherapy for mental, emotional, addictive, behavioral and developmental disorders, and conditions within the scope of practice for a LCSW
- Intervenes appropriately in crisis situations, applying crisis theory and skills
- Participates in the initiation, development, and maintenance of clinical pathways/protocols of population-based programs.
- Serves as a resource for team members and physicians
- Document appropriately and timely in the EMR consults, interventions, care plans and patient outcomes

Education Requirements:
Master’s in Social Work
Licensure/Certification Requirement:
LCSW
Professional Requirements:
Broad clinical experience in an acute or ambulatory care setting is preferred. Excellent communication and interpersonal skills, knowledge of care management and quality improvement principles. Computer skills to be sufficient and effective at utilizing computer applications and EMR.
Through a grant funded partnership this position is eligible for a $5,000 sign on bonus with a 24 month continued employment agreement.
