Treatment is based on the belief that, through a combination of medically managed care and therapeutic intervention and education, patients can learn to effectively manage their illnesses and cope with the effects on their lives.   Location Signature Psychiatric Hospital - Liberty   Full-Time/Part-Time Full-Time   Exempt/Non-Exempt Non-Exempt   Shift Days   Position Social Services Case Manager   Description The Social Services Case Manager assists patients with inquiry calls, coordination of care and discharge planning. Includes gathering patient information, setting follow up appointments, facilitating transition of care to other providers and networking with agencies to better understand their services and referral processes. Additionally the Social Services Case Manager has responsibility for ensuring the proper utilization of hospital resources for patients admitted to the hospital. This position may require some evening and weekend hours as needed to meet patient needs.   Position Requirements Education: Bachelor’s Degree from an accredited college or university, in Social Work, Counseling or a clinical related mental health field. Experience: One (1) year experience in mental health preferred. Additional Requirements: CPR certification and Handle With Care (HWC) during orientation and prior to assisting in a restraining procedure. May be required to work flexible hours and overtime. SKILLS: Ability to communicate in English, both verbally and in writing. Strong interpersonal skills. Knowledge of a wide range of psychosocial, cognitive and/or developmental disorders.
