The MADC II assists patients with inquiry calls, coordination of care, and discharge planning. Includes gathering patient information, setting follow-up appointments, facilitating the transition of care to other providers, and networking with agencies to better understand their services and referral processes. Additionally, the MADC II has responsibility for ensuring the proper utilization of hospital resources for patients admitted to the Outpatient Program.
The MADC II will collaborate with the CD/Clinical Services Manager to plan and develop strategies to improve quality outcomes for CD residents, and work to increase conversion rates of identified patients with substance abuse problems to Changing Pointe Residential Program. Performs therapy service functions to include assisting patients in meeting their bio-psychosocial needs while enabling them to gain/improve skills needed to achieve their optimal level of emotional health. Includes providing consultation, education, specialized therapy services for patients/families receiving care, crisis intervention, therapy. Provides case management as needed to support ongoing recovery.
