Seeking to hire a Full-Time Care Coordinator in the Hospital Division

Job summary

  • Provides comprehensive care coordination, clinically appropriate linkage and referral recommendations to the community and social support services through person-centered care planning and individual and family support services throughout the transition process.
  • Collaborates with hospital staff regarding optimal individual care, provides education and training and captures outcomes.
  • Assures Hospital Division goals and objectives are accomplished by fulfilling job and team responsibilities, as assigned.


  • Bachelor’s Degree in Social Work, Psychology, Counseling or related field
  • Knowledge or experience with providing case management & discharge planning
