Share Email Opening Title Manager of Case Management - Youth  Description Wyandot BHN is seeking a dedicated Manager of Case Management to be responsible for the overall supervision of our youth case management team! The Manager of Case Management will assist team members with providing exceptional service to youth and families by achieving all agency standards. This position is additionally responsible for coordinating outreach to SED children referred to the organization and will provide intake assessments and diagnosis for individuals entering services. Essential Job Duties: Provides clinical oversight for client’s receiving case management services, including ongoing training on clinical care and clinical feedback for staff and the team. Provides weekly group supervision during team meetings. Assures that services being provided are appropriate to the clients needs. Assures team members achieve productivity, deliver quality services and record services in a timely manner and that the team meets revenue and quality standards. Receives and monitors case management referrals & assigns to case managers accordingly. Completes quality reviews and authorization requests for services as necessary. Participates in treatment & discharge planning for children in PACES services, and discharge planning for those discharged from state hospital, JJA and KAW Valley Center as needed. Provides brief individual or in home family therapy as needed. Facilitates wraparounds as needed. Serves as crisis support to case management clients as needed. Represent PACES on appropriate Center committees. Coordinates and monitors Medicaid waiver services.   Position Requirements Job Requirements: Licensed by the Kansas Behavioral Sciences Regulatory Board, and possess minimum academic credentials for Medicaid outpatient reimbursement for practice at the organization. Excellent interpersonal and communication skills. Familiarity with wraparound, the strength based model of case management, trauma informed care practices, and therapeutic interventions children with SED and their families. Ability to initiate and develop solutions in clinical, personnel, and other job related situations. Ability to provide appropriate clinical and personnel supervision. Ability to provide direction for programming and planning purposes.
