Valley Community Services Board is an equal opportunity employer and a drug free workplace.   Open Date 5/25/2022   Close Date   Position Forensic Discharge Planner   Full-Time/Part-Time Full-Time   Exempt/Non-Exempt Non-Exempt   Shift -not applicable-   Req Number CAS-22-00004   Number of Openings 1   Number Filled 0   Description Valley Community Services Board (VCSB) is looking for a Forensic Discharge Planner to join our team!  Under the direction of the Forensic Discharge Planning Coordinator this position is responsible for adults experiencing serious mental illness at Middle River Regional Jail (MRRJ).  Benefits Health insurance as low as $21.16/month for individuals. Family and dependent plans are also available. Sign-on bonus of $1,500, included in full with the first paycheck. Nine paid holidays. Accrue up to 20 days of paid time off (PTO) per year from year one. PTO is immediately available upon accrual. Participation in the Virginia Retirement System. Professional development assistance provided at no cost. Essential Functions Provide discharge coordination and support for all inmates experiencing a serious mental illness within MRRJ. Provide collaborative discharge protocols for CSB, MRRJ and Department of Behavioral Health and Developmental Services. Plan consumer discharges from MRRJ, including coordinating timely discharge appointments at the VCSB outpatient facility, ensuring proper medication coverage and a seamless transition to the community setting. Screen inmates for serious mental illness upon referral from jail staff. Completes documentation of progress by entering progress notes into EHR in accordance with funding source standards and agency requirements. Assists clients with obtaining and maintaining disability and social entitlements and any associated benefits. Facilitate medication continuity with MRRJ resources as well as completion of resource application for housing, employment, healthcare coverage, and other identified resource needs prior to MRRJ release.   Position Requirements Minimum Requirements Bachelor’s degree in human services field. Thorough knowledge of community systems Valid Virginia driver’s license with acceptable driving record required.
