Seeking to hire a Full-Time clinician for the RISE Program

Job summary

  • Responsible and accountable for providing direct behavioral healthcare services for children, adolescents, and adults.
  • Act as a liaison between the RISE/dual diagnosis programs and local schools.
  • Assesses and evaluates clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer.
  • Provides services and supportive interventions, including case management, linkage to community resources, and provides inter-agency coordination of treatment.
  • Documents services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated.
  • Plans for, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum.
  • Functions as a member of an interdisciplinary team in the assessment and/or evaluation of clients, articulates in written documentation the integration of client history and assessment information and actively participates in program and clinical team meetings, provides support for team colleagues, and engages in program-related performance improvement processes and activities.
  • Assists client with concrete services and advocacy, interfaces with other systems on behalf of client, provides, engages in inter-organizational care coordination, and follow up on referrals when indicated by initial and on-going assessment.
  • Assists with problem solving for individual and family member treatment and supportive interventions.
  • Provides educational/supportive group presentations and discussions with clients and/or family members focused on mental illness, chemical dependency/addictions, and/or dual diagnosis.
  • Collaborates with clinicians and administration from local middle schools, high schools, and colleges/universities to establish relationships, coordinate care, and market adolescent and young adult programming

     

Requirements

  • Master’s degree in social work or other behavioral health discipline from an accredited college or university
  • Two (2) to four (4) years of clinical experience
  • MCO panel eligible, as applicable
  • Documented knowledge of a least one (1) year work experience with children and adolescent populations and/or substance abuse counseling when required
  • When Prof. License is pending, employee must be supervised by LCSW/LMFT/LPC until Prof. Lic is received; or
  • Have a Prof. License as required by program’s requirements (e.g.: LAC, LSW, and/or LCADC)
  • A valid driver's license and must be insurable by agency's automobile insurance.
