We are currently searching for an experience RN to support our home health care as a Patient Care Coordinator. The Patient Care Coordinator is responsible for the work flow coordination within Home Care Home Base. In addition, PCC must be able to do an assessment and evaluation of patients. The Patient Care Coordinator assures compliance with agency standards and state and federal laws.
