Mission & Vision:

Job Description:

  • Will oversee RRH Case Management services with direct supervision to case managers.
  • Will apply the Housing First approach to serving people with housing and other service needs.
  • Will participate as an active member of The Louisiana Balance of State Continuum of Care (LA BOSCOC).
  • Will submit routine reports and outcome data, per contractual requirements, and will utilize HMIS system.
  • Will oversee necessary assessments, complete mandated legal documentation, and complete Individualized Service Plans (ISP) according to funding source and policy timelines.
  • Will plan, schedule, and coordinate meetings with RRH Case Managers, their participants, and other providers.
  • Will research, access and link to appropriate medical, psychiatric, education, community and other social services/resources, as indicated by assessments and individual participant preferences.
  • Will communicate and work collaboratively with other providers, leasing managers and/or landlords to ensure appropriate service provision.
  • Will meet with supervisor for regularly scheduled meetings and as assigned by supervisor.
  • Will attend and actively participate in regularly scheduled staff meetings and as assigned by supervisor.
  • Will attend and actively participate in required trainings, as assigned.

Accountabilities:

  • Oversee work provided to the participant, his/her identified support systems to set long-range and short-range goals based on the ISP, ensuring professional recommendations including but not limited to physician, psychiatrist, counselor/therapist(s) and other service provider(s) are incorporated into goals and outcomes.
  • Ensure delivery of services meets the established intensity and frequency as indicated by ISP, funding source or supervisor assignment.
  • Ensure performance benchmarks are met or exceeded for units generated.
  • Ensure documentation and required reports are completed within timelines as required by funding source, policy and supervisor.
  • Make referrals to appropriate medical, psychiatric, education, community resources and other social services agencies and/or providers in a timely manner to address identified needs.
  • Document and follow-up in a timely manner to ensure services are in place and appropriate.
  • Establish and maintain ongoing rapport with participants and his/her identified support systems to assist participants in achieving his/her identified goals.
  • Establish and maintain ongoing rapport with referral sources, other community agencies and providers to coordinate services for participants.
  • Respond to referrals and requests for services within timelines as required by funding source, policy and supervisor.
  • Empower and encourage participants and his/her identified support systems to be active partners in services.
  • Ensure all participants, and their significant others are treated with respect and dignity; and to ensure their rights are preserved.
  • Maintain a minimum 40-hour work week.
  • Actively participate in case reviews, team meetings, staff meetings and other trainings as required by funding source, policy and supervisor.
 
Position Requirements

Qualifications (please take note, we have no flexibility on degrees accepted. These are conditions of our contractor):

  • Bachelor’s Degree in Social Service discipline.
  • A License to practice mental health counseling (e.g. LPC, LCSW); preferred though not mandatory
  • Must have own reliable transportation.
  • Willing to travel during the day to outlying areas if needed.
  • Able to multi-task and meet deadlines.
  • Excellent written and verbal communication skills.
  • Proficiency in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate.
  • Must be a team-player.
  • Easterseals Louisiana is a Drug-Free Workplace, pre-employment and random drug tests will be administered.
  • Will be required to pass Easterseals Louisiana background check

Requirements:

  • The employee is regularly required to operate a computer, file and retrieve written documents,
  • Communicate with others on the phone and in-person.
  • The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items.
  • Ability to move independently within the facility and community.

Salary: $50,000-55,000
