Description

Social Worker needed for full-time, temporary position in Delware County.

The social worker will be primarily responsible for meeting the psychosocial needs of residents residing in the skilled nursing facility. and for performing assigned social service functions in an efficient and timely manner while reporting directly to the Nursing Home Administrator.

Responsibilities include but may not be limited to:

  • Assist in the initial resident admission process, including assisting in transportation arrangements from the hospital, notifying staff of admission, contacting family members before the resident admission, and completing admission orientation with resident and family members.
  • Completing the initial Social History, Geriatric Depression Scale, Mini-Mental examination, and MDS assessment within 14 days of admission.
  • Completing required documentation/progress notes in conjunction with annual, quarterly, and significant change MDS assessments.
  • Participate in weekly Care Conference. Complete individualized care plans following the resident's behavioral, emotional, and psychosocial needs.
  • Assist residents and their families with the transition process through levels of care (Independent Living-Personal Care Unit).
  • Participate in weekly Medicare Utilization Review Meeting. Educate residents and family members concerning Medicare coverage.
  • Coordinate discharge plan of care with residents and their family members. Arrange for home care services and medical equipment needs before discharge.
  • Provide active listening therapy with residents and families who express the need to speak with a supportive individual.
  • Facilitate referrals to the psychologist and/or psychiatrist as needed.
  • Attend Resident Council meetings as a liaison to listen to resident concerns, clarify issues, and/or facilitate change.
  • Facilitate and/or co-facilitate resident and family support groups to provide ongoing education and support.
  • Assist in coordinating admissions from outside resources and the overall management of occupancy within the skilled nursing facility.

 
Position Requirements

BSW, MSW preferred with strong organization and customer service skills. Strong verbal and written communication skills. Ability to prioritize to meet deadlines. Ability to work independently and as a part of a team. Knowledgeable of Medicare guidelines and familiarity with MDS documentation. Ability to develop rapport with residents, their families, and staff. Background and experience commensurate with long-term care services and/or geriatrics. Demonstrated interest, skills, and abilities in working with and understanding the needs of aging adults.
