Share Email Opening Title Part Time OR Full Time Case Manager- CSP La Crosse and/or BRF  Description The Community Support Programs in La Crosse and Black River Falls are looking for a Full Time or two Part Time Case Managers to join our teams. This position is a unique opportunity for an experienced social worker or new graduate seeking to use their skills in a highly collaborative setting working with adults and providing mental health support. Black River Falls: PART TIME, 16-20 hours/week, weekdays only. La Crosse: PART TIME, 24 hours/week, weekdays only. Looking for a FULL TIME position? You can work in both locations: 3 days in La Crosse and 2 days in BRF. Mileage Reimbursement will be provided and competative agency benfits!   Case Management duties include: Providing comprehensive mental health services to adults with severe and persistent mental illnesses, including but not limited to: Assessment, treatment, and discharge planning Implementation of services as needed to assist the client in meeting their recovery plan goal Assisting the client in managing psychiatric symptoms, medical issues that co-occur with mental health issues and substance use issues. Ensuring the client’s basic needs are met Attending all scheduled treatment meetings Provide services in the communities and homes of clients. Completion of all client service related paperwork. Provide case management services as necessary to clients in Lacrosse and/or Black River Falls. Provide after-hours crisis services to CSP clients by phone.   Position Requirements Candidates must have a Bachelor’s degree in a behavioral science or a related field, other fields will be considered if candidates have mental health experience. Preferred but not required: 1,000 hours of supervised post-degree clinical experience with chronically mentally ill persons, or 2,000 hours (bachelor’s degree in a field other than behavioral sciences) of supervised post degree clinical experience with persons with chronic mental illness.  Candidates must be familiar with the Human Service system, recovery principles, and appropriate ethics and boundaries.
