Multiple positions available to work with at their 10 bed hospice facility or in their office and patients homes in Rockland and Orange counties. The social worker provides the patient and family with family counseling and education, psychosocial assessments, community resource coordination, planning and implementation of hospice plan of care, support group faciliatation, and supervision of MSW students. The position is full-time and can be temp, temp to perm, or permanent (permanent is desired by the agency).
