Community First Health Centers is currently looking for a Community Health Worker to join our team at our New Haven medical facility under our Homeless Healthcare service line. The ideal candidate for this role will be enthusiastic, motivated and a team player.

Under the supervision of the Homeless Healthcare Program Manager, this position will work closely with the inter-disciplinary clinical care team to help increase access to health services through outreach, encouraging engagement, education and peer support. This position will be located out of our New Haven medical facility located at 58144 Gratiot Avenue.

Daily responsibilities of our Community Health Worker may include:

  • Promote and maintain awareness of Agency programs, services, and activities
  • Perform initial assessment based on specified reason for care needed through call(s), mail, in-person visit(s) and/or any care setting or community event(s)
  • Coach and motivate patient(s) to engage in primary care to effectively manage their chronic disease or engage in preventative care
  • Address social determinants of health (SDOH) issue(s) identified during patient contacts and eliminate barriers to care as appropriate
  • Conduct SDOH screening(s) and assessment(s) to uncover needs, connect with community resources addressing food, housing, transportation, economic stability, social, community/environment and healthcare
  • Review monthly outreach list and develop a plan to prioritize outreach, documentation of all outreach attempts and provide results
  • Provide direct education and/or engagement with patient(s) and/or connect to available resources
  • Assist patient(s) with changes to their PCP, attempt to schedule PCP appointment(s) and archive those no longer served by our Agency
  • Assist new patient(s) including sharing Agency information, services and what to expect in their initial visit
  • Contact high risk patient(s) weekly to complete risk assessment, engage in their care management and coordination of services
  • Contact high and/or inappropriate utilizers of the ED, encourage PCP engagement, eliminate barriers to PCP care, provide education, resources and encouragement
  • Timely completion of all forms, care recording and required reports including all documentation of outreach attempts, service(s) and/or education provided, referral monitoring and all other pertinent program data
  • Work collaboratively with all members of the clinical care team
  • Builds and maintains positive working relationships with community partners, outreach services, etc.
  • Other duties as assigned

 

 

This is a full time; 40 hours per week opportunity.

Hours will be gained Monday thru Friday; 830am to 5pm and will require some flexibilty based on business demands.

Position Requirements
  • High School Diploma or GED required
  • Experience in community work, education and/or health care required
  • Successful completion of the paraprofessional CHW training program within six (6) months of hire
