This is a full-time position located in St. Louis County. This position could be temporary, temp to perm, or direct hire based on applicants' interest. Hours are typically 8:30-5:15. Home visits are a necessary component of the position, and mileage is reimbursed. Tasks will include: Completing comprehensive assessments which identify medical, psychological and social factors contributing to client's situation Developing an individualized care plan with each client based on identified needs from the assessment and client input Providing resources and referrals Recording client interactions, care plan updates and community resource information Providing outreach and training to community agencies, congregations and organizations   Position Requirements   This position is currently not accepting applications.
