About the Organization Established June 15, 1971, Valley Community Services Board is a publicly funded organization where citizens of Staunton, Waynesboro, and Augusta and Highland Counties can receive Behavioral Health, Substance Use and Developmental Services services. Valley Community Services Board serves as the single point of entry into publicly funded mental health, developmental, and substance use disorder services for these localities.

Description

Valley Community Services Board (VCSB) is looking for a dynamic individual to join our Assertive Community Treatment (ACT) Team. The ACT Co-Leader provides intensive case management, rehabilitation, and supportive services to individuals experiencing serious mental illness. The ACT Co-Leader will work with a population of adults who may be dually diagnosed with chronic mental illness and substance use issues. Services are provided in accordance with the PACT model of treatment and in multiple settings including in the home and in the community.

Essential Functions

  • Provides ongoing consumer assessment, outreach and advocacy.
  • Provides routine community and home visits to assist consumers as defined in their individual treatment plan.
  • Provides leadership to the ACT team in the absence of the ACT Team Leader.
  • Use person-centered treatment planning which reflects consumer strengths and preferences, as well as barriers to successful recovery and community living.
  • Provides individual, group and family rehabilitative and support counseling.
  • Provides emergency/crisis intervention and coordinates for hospital prescreening. Assists with ECO/TDO process and coordination and Emergency Services staff
  • Provides clinical direction and coordination of the team in the absence of the ACT Team Leader.
  • Coordinates with Western State Hospital (WSH) Discharge Liaison for any consumers current inpatient being referred to ACT or currently on ACT census.
 
Position Requirements

Minimum Requirements

  • Master's Degree in human services field and one year direct clinical experience with mentally ill population.
  • Valid Va. drivers license with an acceptable driving record.
  • LMHP or LMHP-type is required.

Preferred Qualifications

Knowledge of

  • ACT model of care.
  • MH & Co-Occurring Disabilities.
  • Person-centered individualized treatment planning.
  • Supportive counseling techniques as it relates to the MH/SA population

Salary Information Up to $51,301/year
