We began in Watsonville 1978 as a storefront “free clinic,” and with the ever growing need for health services, in 1992 became a federally qualified community health center (FQHC). We are seeking an experienced, well organized Comprehensive Prenatal Health Worker (CPHW) to coordinate and deliver patients’ education and access to specialty care providers and services for their prenatal needs. Completes health education, nutrition and psychosocial assessments and delivers health education, nutrition and other educational instructional services to the prenatal patient population. Responsibilities include, but are not limited to, the following: Orients patients to Salud’s Prenatal Program services provided, OB health team, and different types of appointments, locations, and emergency services. Provides health education information in both direct individualized sessions or in group settings. Completes patient assessments, develops individualized care plans for pregnant women and implements interventions and follow-up services in the areas of: Prenatal, Postpartum, Parenting and Newborn Care, Nutrition, Psychosocial and Health Education. Conducts pregnancy tests and provides patients with information regarding all pregnancy options and requirements for Medi-Cal. Acts as a community service liaison and provides community resources and referrals to outside organizations such as WIC, Dental Care, Genetic Screening, Family Planning and Well-Child Care. Assists with individual care coordination and case management. Coordinates with licensed staff such as MD, NP, PA and midwife in dispensing Prenatal Vitamins and ordering prenatal labs. Assists with and orients patients to WIC and Disability programs and forms. Orients patients to CPSP services available throughout their pregnancy. Delivers patient care at all of Salud’s OB clinics; assists with hospital tours and registration; and makes hospital rounds on Maternity Floor. Ensures a productive work environment and achievement of goals through strategies and practices in line with the CARE fundamentals of communication. Performs other duties as assigned. The ideal candidate will embrace our mission to provide high quality health care to patients of all ages, possess excellent communication and problem solving skills, and be familiar with medical terminology and community health care resources.   Position Requirements MINIMUM QUALIFICATIONS High school graduate or equivalent. At least one year of full time paid practical experience in providing perinatal care. “Perinatal Care” includes care provided before, during and up to 60 days following delivery. This candidate would be someone who has provided health information, instruction, counseling or education to women during the perinatal period. CPSP Certificate or acquire within 3 months Six to twelve months experience in CPSP preferred Experience in care coordination and case management Current CPR certification Fluent in all aspects of the English and Spanish languages. Valid California’ driver’s license and current vehicle insurance PHYSICAL DEMANDS: Standing, walking, sitting, typing, reaching, bending, moving and/or lifting up to 25 pounds. SALARY & BENEFITS Salary Range: $19.76 per hour to $24.76 per hour.
