The Social Services Coordinator provides medically-related social services to an assigned caseload that assists the residents to attain or maintain the highest practicable physical, mental, and psycho-social well-being. Services provided meet professional standards of social work practice, consistent with state and federal laws and regulations. Guides facility staff in matters of resident advocacy, protection, and promotion of residents' rights.   Position Requirements BSW with one year of supervised long-term care/nursing home social service work experience working directly with individuals in a health care setting. Demonstrates assessment skills sufficient to collect data and identify psychosocial needs and residents' responses to interventions.   This position is currently not accepting applications.
