Description

POSITION SUMMARY

Incumbent serves as a Referral Coordinator for Tuba City Regional Health Care Corporation (TCRHCC) Ambulatory Care Department. Incumbent is responsible for coordinating care for patients requiring further evaluation(s). Incumbent is responsible for referral operations for the Ya’ateego Iina Pathway Program (YIPP) and referrals to outside network organizations. Incumbent is responsible for establishing and standardizing systems and procedures for the distribution and use of health information throughout the organization. This is a grant funded position.

ESSENTIAL FUNCTIONS

  1. Initiates referral process and evaluated requirements for each referral per protocol.
  2. Assists with the referral appointment setups and maintains follow up of all referrals received and generated within the YIPP Program. Assists with callbacks to patients and outside service providers to whom referrals are generated for patients.
  3. Ensure that referrals are addressed in a timely manner, prioritizing urgent items appropriately i.e. Mental Health, Behavior Health Services.
  4. Assists with the development of a resource directory to ensure referred patients receive patient care services in timely manner. Must be able to identify the requirements of outside service providers in efforts to help patient enroll and receive care.
  5. Provides follow up to ensure continuity of care and monitors patient care plan, as needed. This would be a collaborative effort with Patient Navigator and departments that receive the referral.
  6. Collaborate with Patient Navigator to promote patient self-advocacy and self-determination.
  7. Develops reports and participates in performance improvement projects and attends staff meetings.
  8. Maintain ongoing tracking and appropriate documentation on referrals by entering all referrals into electronic health record, known as Allscripts.
  9. Provides feedback of workflow improvements and uses sound judgement to coordinate referral services for patients.
  10. Assists with the collection of data and must be able to share progress of data collection obtained.
  11. Ensure proper PPE is always worn while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering face piece respirator or higher, if available), and eye or face shield.
  12. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates)
  13. Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee, and external customer satisfaction
  14. Performs other duties as assigned. 

MENTAL AND PHYSICAL EFFORT

Physical:

Incumbent must be able to work prolonged (3/4 workday) sitting and occasionally (less ¼ workday) standing, walking; and, kneeling. Must be able to occasionally (1/4 workday) lifting and pushing/pulling up to 10lbs. Work requires extensive periods of time looking at a computer monitor. Must have ability for near vision, seeing fine details, hearing normal speech, hearing overhead pages over a loudspeaker, and using a telephone. Must be able to work long hours using a computer keyboard and mouse.

Mental:

Contacts often require tact and sensitivity as those related to stressful situations such as death, dying and catastrophic illnesses. Must be able to prioritize and use good judgment and coordinate a variety of issues while being frequently interrupted. Incumbent must have ability to concentrate, handle multiple priorities in stressful situations, work alone, demonstrate high degree of patience for prolonged (3/4 workday) periods of time. Must also occasionally (less than ¼ day) cope with high level of stress, make decisions under high pressure, and, work in areas that are close and crowded.

 
Position Requirements

NECESSARY QUALIFICATIONS

Education:

  • Must have a High School Diploma or GED

Experience:

  • Must have one (1) year of referral coordinator experience or similar job functions/roles

Certification:

  • Must have and maintain current BLS certification by the American Heart Association, if none, obtain within 3 months from date of hire

Other Skills and Abilities:

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

  • Reliable and dependable; reports to work as scheduled without excessive absences
  • Effective verbal and written communication skills

  • Successful completion of and positive results from all background and reference checks, including positive employment references from authorized representatives of past and current employers demonstrating to the satisfaction of TCRHCC a record of satisfactory performance and that the applicant can perform the essential functions of the job
  • Successful completion of fingerprint clearance requirements, physical examinations, and other screenings indicating that the applicant is qualified to be employed by TCRHCC and demonstrating to the satisfaction of TCRHCC that the applicant can perform the essential functions of the job
  • Submission of all required employment-related documents, applications, resumes, references, and other required information free of false, misleading or incomplete information, as determined by TCRHCC
