Position Nurse Practitioner  

The nurse practitioner shall, under the appropriate direction and supervision by the collaborating physician, provide primary health care services at a designated primary practice location. Nurse practitioner may be assigned to another medical practice owned by the Hospital as needs arise and at the direction the CEO and/or Vice President Clinic Operations. Nurse practitioner shall devote efforts to the development of Practice, including being visible in community events and activities, providing community education programs, and other activities to promote Practice. Nurse practitioner will perform duties consistent with the needs of the communities served, in a manner that reflects the Mission, Vision, and Values of Perry County Memorial Hospital.

Position Requirements


Must have a Master’s degree from an accredited School of Nursing. Must assume responsibility for maintaining competence in clinical practice through self and supervisory review, participation in educational activities, maintenance of state licensure, and certification by professional organization.



Must be licensed to practice as an Advanced Practice Nurse in the State of Indiana with authority to prescribe legend drugs, including controlled substances. Must also possess an Indiana Controlled Substances Registration and a Federal Drug Enforcement Administration Registration. Must be current in CPR.



Must have at least one year of clinical experience as a nurse practitioner, either in the nurse practitioner program or since graduation.


Special Knowledge

Internal contacts with staff, patients and family members. External contacts with physicians, office personnel, hospital staff, as well as patients and their families. Tactfulness, courtesy, patience, flexibility and willingness to learn new skills, and the ability to work cooperatively with others required on an ongoing basis to adequately perform in this position. Maintains confidentiality in all matters, discusses patient data or other related work information only with physician or other authorized personnel in appropriate setting. Utilizes initiative and good judgment in adapting or devising techniques and procedures to meet the needs of the office, patients and hospital. Knowledge of state and federal licensure/certification and accrediting agencies required. Knowledge of acceptable medical standards of care is required. Ability to assess, diagnose and implement a plan of care is necessary. Must be able to problem solve and make decisions. Communication skills necessary at all levels. Function within the legal boundaries of a nurse practitioner and shall have and utilize knowledge of the statutes and rules governing a nurse practitioner
