Make out daily assignments for resident care and assure completeness by end of shift. Make daily rounds of all residents and areas, recognizing, interpreting and reporting conditions to the Clinical Manager and documenting same with the assistance of the aides. Make rounds with physicians; call as needed. Administer medications, oral and IM as assigned daily. Do treatments as ordered and routine tasks such as BP's, weights, and intake/output as necessary. Order medications, diets, supplies, and equipment as needed. Complete admission and discharge assessments on all residents as necessary. Conduct team conferences at daily report as needed. Check equipment for cleanliness and condition, med room, linen storage areas, and central bathing areas. Teach and instruct nurse aides whenever necessary. Assist in hands-on resident care. Answer call bells as needed. Monitor the requests/results of laboratory testing and or fax, call abnormal readings to physicians. Make sure all reportable incidents and hospitalizations get reported to the Personal Care Managers immediately. Maintain emergency equipment and supplies. Put demographics and medications in the computer for admissions and medication changes. Assess residents when ill or in pain and take immediate steps to resolve. Update families of residents when there is a change in status as is appropriate. Make rounds two times a shift and PRN in Aspen. Schedule coverage and cover call offs. Communicate resident needs and care levels to outside agencies. Monitor and complete wound care. Train new employees, nurses and unit clerk. Support resident physicians in the medical suite. Conduct audits of the medication carts and resident records. Abide by the organizations code of conduct in the performance of their duties. Perform other related duties as required. Physical Demands: Ability to grasp, kneel, pull, push, reach and stand adequately to perform the essential functions of the job. Ability to lift weight in excess of 50 pounds. Ability to feel, hear, see, smell and speak adequately to perform the essential functions of the job. Ability to work in inside environmental conditions. Along with an excellent work ethic, successful candidates will have: Graduate of an accredited Licensed Practical Nursing School with current licensure by examination in the Commonwealth of Pennsylvania.
