Job Summary

Provides community and public health nursing services in the clinic, community and patient homes in accordance with established policies and procedures with an emphasis on education and prevention. Duties include adult and child care clinics and communicable disease control (i.e., STD, HIV/AIDS, etc.) Patient referrals/follow-up, home visits, individual and group conferences, health promotion and illness prevention activities, patient health counseling, assessments to determine and appraise public health status and needs. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements.

Essential Job Functions

  1. Develop annual schedule for community health screenings, work closely with all entities or event organizers for scheduling.
  2. Must demonstrate and maintain a healthy lifestyle.
  3. Conduct monthly community health screenings.
  4. Assist with public health template development and modifications in the electronic health system.
  5. Work closely with the assigned primary care provider team. Attend weekly huddles, coordinate care closely with medical provider and nurses, and ensure consistent communication regarding patient health status and outcomes.
  6. Must maintain confidentiality in accordance with Yellowhawk Tribal Health Center Personnel Policies and Procedures Manual, HIPAA and the Privacy Act.
  7. Represents Yellowhawk with a respected and viable presence in the community.
  8. Assist with monitoring the high-risk elders list monthly and ensure distribution to appropriate individuals.
  9. Provide home visits as referred by primary care teams; determine frequency and regular schedule per patient/provider needs.
  10. Participate in the planning of health education campaigns and disease prevention activities that involve improving access among community members to clinical care services.
  11. Ensure vulnerable and at-risk populations are receiving education and direct health care as services as appropriate.
  12. Ensure the delivery of communicable disease services including case investigations and contact tracing in alignment with Oregon Health Authority recommended investigative guidelines.
  13. Collaborate with community partners to improve health in the CTUIR community
  14. Use evaluation results and performance management systems to improve programs and organizational performance
  15. Assess, implement and evaluate nursing case management
  16. Assists team with immunization management and coordination, to include providing immunizations upon request or during immunization clinics
  17. Uses public health surveillance/disease investigation methods in community outreach, screening, and case finding of communicable and infectious diseases that threaten the health of the community.
  18. Delivers targeted, culturally-appropriate information to help individuals and groups understand local environmental public health policies, regulations and code.
  19. Utilizes appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds, sexual orientations, lifestyles and physical abilities.
  20. Works with the Public Health Operations Supervisor to educate local and state policy makers and community stakeholders on public health issues which are relevant to Confederated Tribes of Umatilla Indian Reservation.
  21. Adheres to ethical principles and Yellowhawk Tribal Health Center policy in the collection, maintenance, use, and dissemination of data and information.
  22. This position will be responsible for using quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of the respective public health program. This includes, but is not limited to: creating. Implementing, and evaluating performance standards and identifying, implementing, and assessing program quality improvement processes.
  23. Develop monthly reports for appropriate departments and Management teams.
  24. Other duties assigned.
 
Position Requirements

Minimum Qualifications

  1. Registered Nurse with an Associate’s Degree in Nursing with one year successful experience and/or training in Public (or Community) Health Nursing.
  2. Must have active, current license as a professional nurse in the state of Oregon.
  3. Demonstrated ability to communicate effectively, both in writing and oral, in any situation, with peers, staff, management, local, state and federal agencies and the general public, on various topics including but not limited to policies, procedures and general operations.
  4. Demonstrated ability to work independently with minimal supervision, plan and organize work schedules, identify problems and recommend solutions.
  5. Demonstrated ability to handle conflict and crisis in a professional manner.
  6. Demonstrated experience with computer software, e.g., Microsoft Office, Electronic Health Records, RPMS.
  7. Demonstrated excellent interpersonal skills.
  8. Must have and maintain CPR training certification.
  9. Must possess a valid state driver’s license and meet and maintain insurance requirements to operate General Services Administration vehicles.
  10. Must submit to and pass a reference and criminal background check.
  11. Must submit to and pass a pre-employment drug and alcohol screening.
  12. Must submit proof of COVID-19 vaccination.

Preferred Qualifications

  1. Registered Nurse with BSN Degree and training in Public (or Community) Health Nursing preferred.
  2. Experience working out in the community as well as in the patient’s home.
  3. Experience and/or knowledge of history, culture and health needs of Native American communities preferred.
  4. Experience in a Tribal healthcare system
