Serves as liaison between the treatment team and external stakeholders including client families/guardians, funding agencies, agency supports, and involved in child-serving systems including the family court. Responsible for orienting the family/guardian to CIDDS during admission, relaying expectations for family engagement/involvement while a child is in care, and completing a Biopsychosocial assessment. Participates in client treatment team meetings and reports on family engagement, supervised family visitation, family court, and care coordination activities that facilitate current treatment and discharge planning. Documents all family communication/engagement and care coordination activities. Ensures discharge summary is completed promptly. Works with supervisor to improve family engagement and client care experience at CIDDS and to develop support systems and community linkages that facilitate successful discharge. Participates in treatment team activities related to supporting clients and staff in the general milieu and attending training and team meetings.   Position Requirements BSW is required and six months of experience in Social Service is preferred. Proficient computer skills, mastery of Microsoft Office Applications. Strong organizational skills. Excellent oral and written communication skills. Requires ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines.
